Instructions

A 10% deposit is required to reserve a date, preferably with a credit card, then a check for another 40% should be sent no later than one month from acceptance of contract, the remaining 50% is due 45 days before the date of the event. Please enter the TOTAL rental amount in the space provided and add the amusement tax and service fee when sending payment. Remember to sign the contract and fax it to 773-472-1222.
